Micromegaly or acromegaly? A retrospective longitudinal study on clinical aspects and comorbidities in a large cohort of patients referred to a single tertiary center

Carosi Giulia , Sala Elisa , Mangone Alessandra , Del Sindaco Giulia , Mungari Roberta , Cremaschi Arianna , Ferrante Emanuele , Arosio Maura , Mantovani Giovanna

Introduction: The diagnosis of acromegaly is confirmed in the presence of high IGF-1 levels and inadequate suppression of growth hormone (GH) after glucose load. According to guidelines, a GH nadir (GHn)>0.4 ng/ml is considered diagnostic with ultrasensitive assays. However, some acromegalic patients with lower GHn, also called ‘micromegalic’, are reported, but a systematic collection of their clinical features is unavailable.Aim of the s...

Epidemiology of acromegaly in Italy

Malchiodi Elena , Arosio Maura , Borraccino Alberto , Reimondo Giuseppe , Zaggia Barbara , Montefusco Laura , Maria Colao Anna , Terzolo Massimo

Aim of this study is to present preliminary epidemiological data on a population of 1512 acromegalic patients attending 22 tertiary centers in Italy from 1980 to 2002. At our knowledge this is the first large–scale epidemiological Italian study on acromegaly. Gender was 624 (41.2%) men and 888 (58.8%) women. Mean age at diagnosis was 45±13 years, 43±13 for men and 47±13 for women. Median estimated duration of acromegaly before diagnosis was 74 months (range...

Adipokine levels in patients with adrenal incidentaloma

Iorio Laura , Morelli Valentina , Coletti Francesca , Della Casa Sonia , Arosio Maura , Corsi Massimiliano , Chiodini Iacopo , Ambrosi Bruno

Patients (pts) with adrenal incidentaloma (AI) may show an increased incidence of several cardiovascular risk factors, as obesity, hypertension, diabetes mellitus and dyslipidemia. As previous data suggested that the increased cardiovascular risk in these pts could be in part mediated by alterations of adipokines, this study was aimed to evaluate plasma IL-6, resistin, CT-1 levels in a wide series of pts with AI.Ninety-three pts (42 males; 51 females; ag...

Natural history of Rathke’s Cleft Cysts: a multicenter experience

Sala Elisa , Moore Justin M , Amorin Alvaro , Carosi Giulia , Harsh Griffith R , Arosio Maura , Mantovani Giovanna , Katznelson Laurence

Objective: Rathke’s Cleft Cyst (RCC) is a common incidental type of sellar lesion, and, depending on size, may cause local mass effects with visual impairment, hypopituitarism, and headaches. In this study, we sought to define the natural history of RCC.Methods: We performed a retrospective study of patients diagnosed with RCC between 2000 and 2016 at Stanford University Hospital, US, and Ospedale Maggiore Policlinico di Milano, Italy. Aim of the st...

Cystic fibrosis-related bone disease in young adults affected with cystic fibrosis awaiting lung transplantation for end-stage respiratory failure

Cairoli Elisa , Eller-Vainicher Cristina , D'Adda Alice , Briganti Federica , Pappalettera Maria , Tarsia Paolo , Arosio Maura , Chiodini Iacopo

Background: Cystic fibrosis-related bone disease (CFBD) is an emerging complication of cystic fibrosis (CF), especially in CF-lung transplant candidates for end-stage disease. Objectives: The assessment of vitamin D levels, bone mineral density (BMD) and morphometric and clinical prevalent fragility fractures in a cohort of young adults affected with CF awaiting lung transplantation for end-stage respiratory failure. Methods: In 42 C...

Mind the gap between cortisol levels measured with second generation assays and current diagnostic thresholds for the diagnosis of adrenal insufficiency: a single center experience

Grassi Giorgia , Morelli Valentina , Polledri Elisa , Fustinoni Silvia , Chiodini Iacopo , Ceriotti Ferruccio , D'Agostino Simona , Mantovani Giovanna , Arosio Maura

Objective: The current cut-offs to diagnose adrenal insufficiency (AI) have been established using outdated immunoassays. More modern methods have demonstrated less cross-reactivity with other steroids. The aim of our study was to evaluate the correlation between the cortisol assay Roche Cortisol I (R1), the newly available Roche Cortisol II (R2) and liquid chromatography tandem mass spectrometry (LC-MS/MS), the gold standard procedure for the measurement of steroids.<p cl...

Effects of adrenalectomy on arterial hypertension, glucose and lipid metabolism in patients with mild autonomous cortisol secretion: preliminary results of a Randomized Clinical Trial

Frigerio Sofia , Morelli Valentina , Aresta Carmen , Elena Passeri4 , Pugliese Flavia , Corbetta Sabrina , Scillitani Alfredo , Arosio Maura , Chiodini Iacopo

IntroductionThe management of patients with adrenal incidentaloma (AI) and mild autonomous cortisol secretion (MACS) is debated. This randomized study aimed to evaluate the effects of adrenalectomy on arterial hypertension (AH), glucose and lipid metabolism.MethodsWe consecutively evaluated 626 AI patients (referred to 3 Italian Centers between 06/2016 and 02/2020). According to the inclusion criteria, we enr...

Metabolic and Inflammation markers in patients with mild autonomous cortisol secretion: preliminary results of a Randomized Clinical Trial

Morelli Valentina , Frigerio Sofia , Perego Silvia , Lombardi Giovanni , Aresta Carmen , Pugliese Flavia , Scillitani Alfredo , Arosio Maura , Corbetta Sabrina , Chiodini Iacopo

IntroductionIrisin is a molecule secreted from skeletal muscle and contributes to maintenance of metabolic homeostasis leading to increased energy expenditure and reducing the risk of obesity and diabetes. Literature data suggest that patients with Cushing disease have lower irisin values than controls and remission of hypercortisolism increases these values. Moreover, other plasma inflammation markers implicated in the pathogenesis of atherosclerosis, i...

Adrenalectomy improves blood pressure and insulin secretion in adrenal incidentalomas and MACS

Morelli Valentina , Musolino Antonio , Palmieri Serena , Pugliese Flavia , Aresta Carmen , Frigerio Sofia , Arosio Maura , Corbetta Sabrina , Scillitani Alfredo , Chiodini Iacopo

Introduction: In patients with unilateral adrenal incidentaloma (AI) and mild autonomous cortisol secretion (MACS) the best therapeutic approach is still a matter of debate. The only available randomized study showed a short-term beneficial effect of surgery on diabetes and hypertension. Importantly, recent data demonstrated that MACS is associated to an increased risk of mortality, especially due to cardiovascular causes.Study aim: to evaluate, in the m...

Splicing factor 3 subunit B1 (SF3B1) inhibition in PRL-secreting PitNETs and cross-talk with dopamine receptor type 2 (DRD2)

Di Muro Genesio , Mangili Federica , Barbieri Anna Maria , Esposito Emanuela , Treppiedi Donatella , Arlati Federico , Catalano Rosa , Marra Giusy , Nozza Emma , Arosio Maura , Mantovani Giovanna , Peverelli Erika

Somatic mutations in splicing factor 3 subunit B1 (SF3B1) were found in about 20% of PRL-secreting PitNETs. SF3B1 is involved in pre-mRNA splicing and required for assembly of the U2 complex, which is critical for branch site recognition and the early stages of spliceosome assembly. Patients with mutant prolactinomas showed higher PRL levels and shorter progression-free survival compared to wild-type patients. Aims of the present study were: 1) to characterize the genetic prof...
